We obtain a comprehensive health history, including past medical history, allergy and medication intolerance information, current medications and supplements, and recent laboratory data. Utilizing this information, we assess the appropriateness of each medication based on safety, effectiveness, and cost. We may also identify additional treatments that may be of benefit. We will also address any questions or concerns you may have about your medications.
As part of our subscription service, we provide you with a personal medication record that includes all of the important information about your medication therapy. You can use it as a personal reminder tool, and bring it with you to all of your appointments to ensure all of your providers have your up to date information. If you are a premium member, just notify us anytime there are any changes, and we'll keep it up to date for you.
After reviewing your information, we will formulate a list of medication issues. These may include considerations for monitoring, dose adjustments, medication changes, and or non-medication related treatments or changes. For our premium members, we update the action plan when any changes in your health status or medications occur.
After formulating your medication plan and allowing you to review our comments, we will contact your health-care provider(s) about any potential medication related problems.
